To apply for this scholarship, please do the following:

1. Read the Requirements to ensure that you qualify.
2. Complete and submit the Scholarship Application form and all required supplemental materials. Applications will be available and accepted beginning November 1, 2011 and must be postmarked no later than February 10, 2012. You will need ADOBE Reader 6.0 or greater to open, use and print the PDF fill-in-the-blank form. If you are having difficulty filling out the form, read the instructions.
3. Take the ACT or SAT I on or before the December test date.

Test scores can be provided by the school official completing the Transcript Information section of the application.

Deadline
The application and all supporting materials must be mailed to Scholarship Management Services and postmarked no later than February 10, 2012 to receive consideration.
